Model’s Full Legal Name (the “Model”), for good and valuable consideration, the receipt of which is acknowledged, hereby grants to Company Name (“Company”), its legal representatives, assignees, and those acting under its authority, the unrestricted right and permission to copyright and/or use, and/or publish photographic portraits or pictures of the Model (the "Pictures"), and the negatives, transparencies, prints, or digital information pertaining to them, in still, single, multiple, moving or video format, or in which Model may be included in whole or in part, or composite, or distorted in form, or reproductions thereof, in color or otherwise, in any media for advertising or any other lawful purpose.

Model hereby relinquishes any right that he/she may have to examine or approve the finished product or products or the advertising copy or printed matter that may be used in connection with an image that Company has taken of the Model, or the use to which it may be applied. Model waives any ownership and publication right he/she might be entitled of in connection with the Pictures, with the exception of the personal use of the Pictures as part of personal modeling portfolio. Model agrees he/she has no right to sell, license or publish any of the Pictures to any person or entity.

Model further releases Company its parent company, subsidiaries, affiliates, officers, agents, servants, or employees (“Releases”) from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the proper commercial or artistic use of these images unless it can be shown that said reproduction was maliciously caused, produced and published for the sole purpose of subjecting the Model to conspicuous ridicule, scandal, reproach, scorn and indignity. Model also waives any and all claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by Model, or any of the property belonging to Model, whether caused by the negligence of the Releases, or otherwise, while performing the photography session, or while in, on or upon the premises where the photography session was performed.

It is the express intent of Model that this document shall bind all members of Model’s family, heirs, assignees and personal representatives.

Model represents that he/she is 18 year or older, that the photography session was conducted in a completely proper and highly professional manner, and this release was willingly signed.

In consideration of something of sufficient value to the circumstances, e.g. money, prints etc., the receipt and  sufficiency whereof is hereby acknowledged by the Model, both parties hereto agree as follows:

DESCRIPTION OF PHOTOGRAPHS.

This Agreement applies to any and all photographs in any format, still, single, multiple, moving or video, (herein collectively called 'the Photographs').of the Model and the Model's Property made by the Photographer on the session dates covered by this Agreement (noted below) and to all reproductions and modifications of such Photographs.

USE OF THE PHOTOGRAPHS.

The Model hereby consents to and authorizes the use of the Photographs by the Photographer and the Photographer's authorized representatives, licensees, successors, and assigns for any purpose whatsoever including and without limitation: sale, reproduction in all media, publication, display, broadcast and exhibition for promotion, advertising, trade, art or illustration. The Model agrees that the Photographs may be used without further compensation for an unlimited time and that this Agreement is irrevocable.

OWNERSHIP AND RIGHTS IN THE PHOTOGRAPHS.

The Model agrees that the Photographs, the copyright in the Photographs and all other rights in the Photographs orcopies or reproductions thereof are the sole property of the Photographer and that the Photographer may protect the copyright or dispose of or authorize the use of any or all such rights in any manner whatsoever.

RELEASE OF PHOTOGRAPHER FROM LIABILITY.

The Model releases the Photographer and all other persons entitled under this Agreement to use the Photographs from all liability for libel, invasion of privacy, and all causes of action whatsoever in relation to the Photographs, their making and use, the Model or the Model's property including without limitation any liability for alteration of the Photographs, whether intentional or otherwise, that may occur during the making or subsequent use of the Photographs.

The Model is not a minor, and does have the legal age or status in his/her country to sign this Agreement and is 18 years of age or older on the Photographs session date(s). The Model understands English. The Model acknowledges reading the entire Agreement in full and attentively prior to signing, and the Model is perfectly familiar with its contents.

This Agreement shall be binding upon the Model’s heirs, legal representatives, and assigns.

8. my Image may be published or distributed by the Photographer, using the Media throughout the world on any website or in any publication, video or film, for commercial or non commercial purposes, without restriction (to the extent permitted by law), as an intermediary product or a finished product. For the avoidance of doubt the Photographer may make the Image available for resale to third parties for the reproduction and/or distribution by that third party in any medium throughout the world;

9. the Photographer is not obliged to include my Image in any website, publication, video or film or for any other commercial or non commercial purpose;

10. other than the publicity I may receive if the Photographer use my Image, no further consideration is due to me for this release;

11. this release is irrevocable, worldwide and perpetual, and will be governed by the law of New South Wales, Australia;

12. I have read the above release and fully understand the contents and consequences of such release and that it shall be binding upon me, my heirs, my legal representatives and my assigns and I willingly sign this release; and

13. I am over the age of eighteen (18) years and hereby warrant that I sign this release without breach of any prior agreements or applicable law including but not limited to prior agreements with modelling or talent agencies or any other person, company or entity.
